Pertemps Exeter are proud to be working with a charity organisation who are currently hiring for an Early Intervention Worker within their outreach team to work alongside their team based in the centre of Exeter.
The purpose of the post is to help people experiencing rough sleeping to move into 'off the streets' placements, such as short-term assessment beds and emergency sleeping pods. You will complete comprehensive strengths-based assessments with these clients and present their case at the weekly 'Access 2 Accommodation' (A2A) meeting where they will be matched to the accommodation vacancy most suitable for their presenting needs. You will also work with individuals in short term placements to minimize abandonment, sustain accommodation, and help people access settled accommodation that ensures they do not return to the streets.
As part of this Early Intervention role, you will deliver high quality, person-centered assertive outreach sessions, working as part of a team, contacting new, repeat and long-term people sleeping rough across the Exeter Council area. You will work with these individuals developing person-centered support plans that empower the individual to address their health and well-being needs
Salary of 23,240 annum
This is a permanent, full-time role working 37.5 hours per week. The role is located in Exeter however due to the nature of the role a valid driver's licence and access to a car for business purposes would be required.
Participation in an Outreach rota which may include working early mornings, evenings and some weekends
Please note, this role will include working weekends and unsocial hours and being able to walk a number of miles in all weather conditions.
Responsibilities include:
To undertake assertive outreach and hot spot visits across the locality area
Handle referrals and support individuals accessing emergency sleeping
Carry out needs assessments, support plans and risk assessments with identified people who are sleeping rough to secure access to services that are suitable for their immediate needs (such as off the street placements), and make appropriate referrals in partnership with them to establish a sustainable life away from the streets
Empower a caseload of clients, to identify their strengths and goals, and develop their skills, confidence and well-being
