Early Intervention Worker
LOCATION: Exeter
HOURS: 37.5hours per weekOn-call (1 in 4 weeks) £100/week when on-call

About the Organisation:
This is a great opportunity for someone who already holds case management experience or knowledge of complex needs, alternatively, someone with a passion for the sector, some initial support experience or a relevant degree will also be great!
Main Activities and responsibilities for the Role:
- Support service users with their sign up, ensuring appropriate tenancy and support agreements are in place.
- Provide outreach support particularly in local areas deemed as hot spots.
- Manage referrals and support individuals accessing emergency sleeping.
- Create, review, and update support plans and risks assessments.
- Make referrals in partnership with external services to find sustainable accommodation
