Job description

Main Purpose of the Role:

Family Carer Practitioners facilitate and empower carers and to achieve the greatest degree of choice, control, independence, and quality of life as possible to maintain their caring role. Practitioners will support carers in their caring role with information, advice and guidance, wellbeing support, access to breaks and peer support opportunities, emergency planning and links into other preventative services. Practitioners will undertake support planning and reviews with carers. They will work with carers enabling them to increase their resilience, build on strengths and develop their personal networks in the community helping them to sustain their caring role. This is a rewarding role with the Carer at the heart of everything There is also a level of computer work involved and so the successful candidate will have a high level of IT skills.

Main Duties:

1. Carers Assessment and Support Planning:

  • Complete reviews with carers at timed intervals.
  • Supporting the carer to identify their progress towards achieving or maintaining goals, developing self-resilience and sustainability of their caring role.
  • Where progress against goals is not achieved or maintained, continue to work with the carer to identify barriers and provide support to overcome them.
  • Work with a person-centred carer led approach to empower them to achieve the goals and outcomes.
  • Support empowerment, independence, social and emotional wellbeing through information, advice, and local opportunities. Encourage use of existing community resources and facilities.

2. Recording, Quality Management/Performance, and Information Governance:

  • Record all interactions, interventions, notes, assessments, and actions for follow-up on the CRM database within the KPI time frame and to expected quality standards, ensuring data is accurate and proportionate, providing carers with a physical or electronic copy of their assessments and Action Plans within agreed timeframe.
  • Contribute to service monitoring and evaluation.
  • Deliver against specific targets and Key Performance Indicators, review progress, and set short term priorities in regular supervision with line manager.
  • Work within the organisational policies and procedures, including but not limited to assessment, support planning/review processes, data input, safeguarding and lone working procedures.

3. Other:

  • Positively participate in the staff development, including service monitoring, coaching and personal development planning, supervision, and appraisal / performance reviews, in line with the performance and quality management framework
