The Children’s Transition team is based in Ellesmere Port and is co-located with wider children’s social care, early help and education teams. The children’s transition team works with young people from the age of 14 up until they are ready to transition into adult social care teams. We work across the whole borough of Cheshire West and Chester so there will be a requirement to travel. The Family Support Worker role promotes independence and life skills with disabled young people and their families with the outcome that disabled young people are prepared for adulthood and to achieve their optimum independence.

Our teams are encouraged to work creatively in planning and decision-making with children and their families in order to achieve positive outcomes and with a focus on prevention at every level.

Strong political and senior leadership team has a wealth of experience and knowledge about children’s outcomes across the boroughs of Cheshire West and Chester. Children’s social care and the wider partnership use this understanding well to target areas for continued improvement.

You will have NVQ Level 3 or equivalent in a related area (e.g.: Children’s Social care, Health) and will need a minimum of two years’ experience of working with young people and families.
