Job overview
FOLS is a community-based service in Sussex providing support to individuals with mental health issues related to their offending/risk behaviour. Based in Sussex, the focus of this exciting role will be to provide support in the community to a small caseload
We are looking for an enthusiastic, dynamic individual, able to demonstrate a strong sense of initiative, adapting to any given situation, who works well as part of a team. A friendly caring attitude, sense of humour. As Lead Practitioner, you will carry out specialist assessments, devise robust risk management plans, whilst implementing bespoke interventions. The role encompasses working as an experienced clinician with service users and also skills in leadership, supervision, service development and improvement. Your role will be diverse, work is varied, fast paced and demanding, so the post holder must be flexible in their practices. We ask you to be committed to delivering a quality service and you are passionate in developing/improving service provision.
We offer, training opportunities whilst you are working in a supportive environment within a specialised team, regular supervision and support, opportunities for continued professional development including bespoke CPD. We will ensure you have a suitable induction period, with training provided according to your needs, along with support and weekly reflective practice from an experienced MDT. We support the option to lease a trust car in our service.
This post offers the Trust's £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).
Main duties of the job
You will be employed as Band 6 Forensic Community Psychiatric Nurse with a full-time substantive contract within the FOLS team, the role will be covering forensic patients in West Sussex .You will be working with patients with offending behaviours, this will include those on restriction orders under the Ministry of justice.
You will provide support with transition from secure hospital into the community.
You will be a Lead Practitioner holding a case load, and providing a specialised range of clinical interventions.
You will monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of interventions.
You will undertake clinical assessment, formulation, intervention, hold CPA reviews
To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and careers
To regularly review risk and feedback to the MDT
To offer specialist nursing assessment and advice where required.
You will adhere to the team and Trust's lone-working policies at all times.

Detailed job description and main responsibilities
To be a core member of the team, using specialist assessments.
To provide a highly specialised range of clinical interventions relevant to the care group and have knowledge of evidence-based models of practice.
To manage a caseload of clients with complex needs.
To work in partnership with the individual service user and their families and careers as appropriate to ensure the delivery of the care plan, and to enable the development of a plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community, by using recognised age appropriate self-management tools where applicable This may include working closely with partner agencies ,voluntary sector agencies, , MAPPA ,which may be able to more appropriately help the service user complete these tools.
To administer and monitor medication prescribed to individual service users, providing information and advice.
To discuss with the service user and when appropriate, with their careers, their health problems and how they see and understand them, facilitating a process of developing their understanding and maintaining their hope for the future.
