This is an exciting opportunity for a social worker or allied health professional to join health and social care practitioners together, to support the prevention of unnecessary hospital admissions / readmission across Manchester; to develop a policy and pathways between relevant partners and offer training and support where necessary to colleagues in different parts of the organisation or health and social care system within the MLCO.
You should be someone with a level of professional experience in supporting those with a Learning Disability and other related issues such as autism, mental health and have some supervision experience of staff / students. A positive approach to risk management, be reflective, solution focused and strength based in their practice. Partnership working should be at the centre of the role and will be one of the key enablers to ensure best outcomes for citizens.
You should be used to working in a fast paced and pressurised health or social care setting being able to manage in this environment; whilst also being focused on performance, management and accountability to the various interested parties across the health and social care system, which would include GM and NHSE.
The team is made up of social workers and Learning disability Nurses, they are based within a wider CALDS (Community Adult Learning Disability Service).
