About the Role:
As a Forensic Social Worker, you will play a vital role in ensuring comprehensive and holistic care for patients within our clients secure mental health setting. Your responsibilities will extend to identifying and addressing social issues, creating meaningful links between the care team and the patient’s community, and actively participating in safeguarding efforts.
Key Responsibilities:
1. Holistic Care Delivery:
– Collaborate with the multi-disciplinary team to develop and deliver a comprehensive care plan for patients.
– Identify and address social issues within risk assessments and care plans, ensuring robust support mechanisms are in place for patients.
2. Community Integration:
– Establish links between the ward’s care team and the patient’s community team to facilitate early discharge planning.
– Develop connections with different agencies within the patient’s local community, including Care Co-Ordinators, MAPPA, families/carers, and potential future placements.
3. Safeguarding:
– Take a proactive approach to monitor and address safeguarding concerns among the patient group on the ward, ensuring the safety of patients at all times.
4. Investigations and Feedback:
– Conduct investigations into incidents within the hospital, providing constructive feedback to the Senior Management Team.
– Investigate and address complaints made by patients in the hospital in collaboration with the Complaints and Compliance Administrator.
5. Support and Community Resources:
– Support carers and family members of patients, guiding them to appropriate resources within their local community for necessary support.
6. Leadership and Inspections:
– Lead the team to meet expected standards in inspections, aiming for a rating of at least “good” in future CQC inspections.
7. Forensic Framework Understanding:
– Demonstrate a good understanding of the forensic framework that underpins policies and protocols within a Medium Secure Hospital.
8. Supervision and Mentoring:
– Engage in monthly supervision with your Line Manager.
– Oversee the mentoring of student Social Workers, arrange student placements, and develop links with local universities.
