A Vacancy at Blackpool Teaching Hospitals NHS Foundation Trust.
We are looking for an enthusiastic and motivated individual to join the friendly and dynamic Community Frailty Team at Blackpool Teaching Hospitals NHS Foundation Trust to develop and embed high quality care for the frail and older patients in varying clinical settings (including community, virtual ward and across the acute interface).
We are looking to recruit a General Practitioner with an interest in Frailty with passion for excellent care to join our supportive team which also includes advanced clinical practitioners, nurse consultants and GPs with an interest in Frailty. New appointees would be supported with a mentor.
We are particularly keen to improve several of our services particularly our Frailty pathway (acute, community and virtual). The post holder will be employed by Blackpool Teaching Hospitals NHS Foundation Trust, based in the community across Moor Park Health & Leisure Centre and South Shore Primary Care Centre.

It is an exciting time to become part of the Blackpool Teaching Hospitals family as we embark on a transformational change journey in the way which we provide services to our patients across our local Integrated Care System.
The post holder will provide evidence based, comprehensive, person centred and individualised medical care for frail people, involving completion of Comprehensive Geriatric Assessments.
You will work as part of a multi-disciplinary team which is led by a Consultant Geriatrician and includes GPs with an interest in Frailty, Nurse Consultants, Advanced Clinical Practitioners, Community Frailty Nurses, Pharmacists, Pharmacy Technicians, Assistant Practitioners, Health Care Assistants and an administration team. The role will also encompass MDT leadership of our Frailty Virtual Ward which is additionally supported by our community Rapid Response Team.
There will be opportunities to participate in developments supporting patients across our acute / community interface especially with our planned development of a new Acute Frailty Unit on our Blackpool Victoria Hospital site. Our aim is to develop a seamless frailty pathway for patients across acute, community and virtual settings.

The post holder will develop specialist frailty knowledge required to support the management of complex and challenging presentations of frail people and the post will prove extremely varied and rewarding.
The post holder will also be able to contribute to service development, local clinical pathway development and implementation for people living with frailty and engage in audit and quality improvement initiatives.
The post holder will be actively involved in training junior doctors (including GPST trainees) and advanced clinical practitioners. The Trust has a state-of-the-art Simulation and Skills Facility which is accessible to students and staff. Should the successful applicant wish to develop a research interest then this would be encouraged.
