Peterborough City Council has an exciting leadership opportunity for an experienced Head of Service working in the Adult Social Care Directorate.
The post-holder will be accountable and provide operational leadership, development, and management for all aspects of Transfer of Care relating to hospital discharge and all aspects of Continuing Health Care ensuring the highest possible quality of Service is delivered within the resources available.
The role is critical in ensuring Adult Social Care works collaboratively with colleagues across the Integrated Care System (ICS) to develop procedures, practice and systems that will ensure effective and efficient service delivery through the Transfer of Care Hub. The post-holder will ensure adherence to the principles of Home First ensuring people are supported to continue to live independent and fulfilling lives within their local community.
The successful applicant will be strategically responsible for improvements, leading and developing practice and cultural change in line with the planned developments of the Home First discharge planning and Continuing Health Care work streams.
The Care Quality Commission requirements in relation to safe transitions from hospital and performance management of social care flows and outcomes from discharge on social care pathways will also be a key focus for this role.
