Job description

To collect, retrieve and enter patient data from a variety of sources in order to facilitate Clinical Audit and data within the Heart Failure Service for the National Institute for Cardiovascular Outcomes Research (NICOR).
To provide comprehensive and high quality secretarial, administrative and clerical support to the Heart Failure department ensuring an effective and efficient service is delivered.
To run and manage the weekly Heart failure MDT meeting.
The successful candidate will work as part of a busy team and will be required to communicate well with peers and colleagues.
They will be required to have an advanced knowledge of excel and other computer systems, with an interest in data and how data can be used to develop a service.
The role will include working with patient notes, so an understanding of medical terminology would be beneficial, with the ability and willingness to learn the specialist’s terminology.
The successful candidate should be an enthusiastic and ambitious person.
  • To provide comprehensive data entry for national and local databases. Ensure these are completed to the required standards and in the appropriate timeframes, contributing to the Trust’s achievement of these targets.
  • To work closely with all grades of clinical and administrative staff, along with other employees within the Trust. To liaise with patients, GP’s and other external agencies.
  • This position requires empathy and tact when dealing with patients that may be angry or upset regarding any aspect of their care.
  • To communicate effectively in writing and verbally on a daily basis.
  • To ensure all results of tests and investigations are tracked effectively, shown to the relevant clinician promptly and to undertake appropriate action as instructed.
  • To independently deal with patient/carer telephone calls and enquiries in an efficient, sensitive and confidential manner, exercising judgement to analyse and resolve any problems and referring on to the clinician as appropriate.
  • All clinic outcomes must be recorded accurately and complete or amend any that have not been done

We undertake assessments and investigations, in addition to all aspects of treatment of patients with diseases and defects of the heart / cardiovascular system.
You will be joining a supportive and dynamic team, who are committed to continuously improve the service for our patients.
  • To collect and input data for patients who are to be included in the National Institute for Cardiovascular Outcomes Research (NICOR) Heart Failure Audit – approximately 450-500 inpatients per year.
  • To identify missing data and use a variety of sources to complete records.
  • Work closely with clinical and admin staff within the cardiology service to reduce the occurrence of missing data and improve data quality.
  • Embed changes and improvements to data collection and reporting systems in collaboration with clinical teams.
  • To ensure data is uploaded in a complete, timely and accurate manner.
  • Adhere to procedures which comply with Information Governance, to ensure data integrity, data backup, disaster recovery and data security, reporting any breaches or concerns as appropriate.
  • Undertake regular validation audits of the databases update and amend where appropriate.
  • To extract, interrogate and manipulate data from manual or computerised records for the purposes of cohort identification for the population of NICOR.
  • Liaise professionally with all departments regarding admissions and discharges to ensure all data is correct.
  • To escalate coding discrepancies within the clinical teams.
  • To develop and maintain effective working relationships with colleagues both internally and externally as required.
  • To work closely with the Lead Heart Failure Nurse and Audit and Performance Data Manager to ensure outstanding work and priorities meet the department and Trust’s requirements in providing data/information/reports requested both locally and nationally.
  • To contribute to the achievement of the Trust’s local and national targets, by working with clinical teams.

  • To ensure compliance with all Trust policies and procedures.
  • To attend all required mandatory training sessions.
  • To ensure that all clinical and non-clinical correspondence is typed within local targets, using the Infoflex system. (Audio and copy typing)
  • Ensure appropriate follow up is actioned i.e. outpatient appointments, investigations, patients added to waiting lists, referrals to other departments etc
  • To review discharge summaries and ensure investigations/appointments are booked appropriately
  • To ensure that all results/reports/correspondence are filed in the hospital case notes in accordance with Health Records standards
  • To identify areas for service improvement and support implementation
  • To support any Secretarial Assistants within the department in their role and ensure compliance with Standard Operating Procedures
  • To comply with the Trust’s Patient Access Policy and Standard Operating Procedures
  • To enter, monitor and update all aspects of the Referral to Treatment (RTT) pathway events accurately and ensure Inter Provider Transfers (IPT) are completed and updated. Review statuses are to be updated at every patient interaction
  • To manage the e-referral worklists on a daily basis
  • To use the relevant IT systems to register patients, respond to enquiries, make appointments, transfer medical records, input clinical letters and care planning summaries, as required
