Exciting Opportunity: Hospital Link Worker
We are seeking a dedicated and proactive Hospital Link Worker to join our dynamic Alcohol Liaison Team. This role is pivotal in reducing unplanned, alcohol-related hospital attendances and admissions by providing assertive outreach to individuals in need.
Key Responsibilities:
- Facilitate engagement with community drug and alcohol services, and support regular attendance at hospital outpatient and GP appointments.
- Identify barriers to change and provide harm reduction interventions in the community, including the service user’s home.
- Develop community links for individuals and manage care planning, risk assessment, and case management for allocated service users.
- Use case management systems to accurately record service user data.
About You:
- Previous voluntary or paid experience in a client-facing role.
- Proficient in using active listening skills on a day-to-day basis.
- Motivated, proactive problem solver passionate about delivering a high standard of care.
- Comfortable with lone working, adaptable to various settings, and able to build rapport quickly.
