We have an exciting opportunity for an enthusiastic Social Prescribing Link Worker to join our dynamic and successful team. This post will specialise in three areas:

Social prescribers may offer enhanced support to individuals who have dropped out of a Cardiac Rehabilitation programme. The individuals may be supported to either:

- Social prescribers will also work with individuals to help them apply what they learn from cardiac rehab to their home lives.

- Offer support to address other issues affecting or impacting their lives, including but not limited to: Housing, Finances, Social Connectedness, Physical and or Mental Health, Employment and more.

3. Long-term maintenance of cardiac health through phase 4 or alternative exercise programs

This role is well suited to individuals with experience of directly working one-to-one with patients/clients, families and carers, to plan and help people resolve practical issues with Housing, Finances, Employment alongside wider Health and wellbeing support, such as managing health conditions by keeping active and connected.
