We have an exciting opportunity for an enthusiastic Social Prescribing Link Worker to join our dynamic and successful team. This post will specialise in three areas:
1. Increasing uptake of Cardiac Rehabilitation
Social prescribers will work with individuals who have been referred to, but have not attended, local Cardiac Rehabilitation services. Where possible, social prescribers will contact individuals to offer them enhanced support.
2. Supporting completion of the Cardiac Rehabilitation programme and the incorporation of rehabilitation principles into everyday life.
Social prescribers may offer enhanced support to individuals who have dropped out of a Cardiac Rehabilitation programme. The individuals may be supported to either:
- Complete the cardiac rehab programme; or
- Find alternative community-based support. Note: where no suitable cardiac rehab programme can be found, the reasons for this should be documented and fed back to the evaluation team.
- Social prescribers will also work with individuals to help them apply what they learn from cardiac rehab to their home lives.
- Offer support to address other issues affecting or impacting their lives, including but not limited to: Housing, Finances, Social Connectedness, Physical and or Mental Health, Employment and more.
3. Long-term maintenance of cardiac health through phase 4 or alternative exercise programs
Social prescribers may work with individuals to identify long-term options to help them stay well and as independent as possible. This may include walking or dancing groups, exercise classes, exercising at home or other health and wellbeing activities to maintain fitness.
This role is well suited to individuals with experience of directly working one-to-one with patients/clients, families and carers, to plan and help people resolve practical issues with Housing, Finances, Employment alongside wider Health and wellbeing support, such as managing health conditions by keeping active and connected. Based within Islington, you will be working closely within Secondary Care and Cardiac Rehabilitation professionals, multi-disciplinary teams, other Health & Social Care professionals, together with other Age UK Islington services.
You will have responsibility for case managing a quota of clients, triaging referrals, actively connecting people to specialist services or community services to promote independent healthy living.
Your role involves working with people who will be experiencing complex issues, so you will need good listening and communication skills and be able to build effective partnerships with a wide range of organisations and community groups.
You’ll need excellent IT and organisational skills, be adept at seeking out the information for relevant community services and have a tenacity to help people achieve sustainable outcomes and improvements in their wellbeing.
This role is subject to an Enhanced DBS check with Adult and Child Barring.
