BASE: Home working and community working across Gwent.
The purpose of this role is to provide high quality support to older people classed as Elderly Mentally Infirm across Gwent. You will have a clear understanding of older people’s needs, especially those who are diagnosed with mental health illnesses.
Support will be person centred, you will actively promote a client’s wellbeing and provide practical support including; the completion of benefit checks and applications to maximise income, shopping, light domestic and signposting to other relevant services to create resilience and to support them to remain living independently in their own home.
You will manage your work time and case load effectively, have a passion for supporting older people and be flexible in order to meet service needs. The ability to drive and have access to of your own vehicle for work purposes is essential to your role. You will also undergo an enhanced DBS check.
