Job Title: Support Worker

DBS Level: Enhanced Adult
Working Hours: 40 hours per week, Between 8 am - 10 pm (8-4, 2-10 pm), 1 Saturday a month

Main Duties:
- Plan and deliver effective support and safety plans with the service user, referring to specialist agencies as required.
- Ensure that properties are always adequately equipped, maintained, furnished, and cleaned.
- Support residents in understanding responsibilities to maintain their CAS-2 and future accommodation.
- Empower and motivate service users to identify and achieve desired outcomes.
- Develop a move-on plan with service users at the earliest opportunity, identifying realistic options for their future home.
Additional Responsibilities:
- Conduct regular assessments to understand service users' evolving needs.
- Collaborate with external agencies to provide specialized support for substance misuse, physical health, mental health, employment, and housing.
- Facilitate group sessions or workshops to enhance life skills and social integration.
- Advocate for service users and ensure their voices are heard in the decision-making process.
- Monitor and report on the progress of service users, maintaining accurate and up-to-date records.

- Ability to work with vulnerable adults with complex needs (such as substance misuse, physical health, mental health, seeking employment, and Housing Support).
