The post holder will support young people in a childrens residential setting and on the edge of care.
The post holder will provide all aspects of care needs and interventions, in line with young peoples care plans, and develop effective action plans with a focus on reducing risks and building on strengths. Key working is integral to the post; working with young people to develop their resilience and life skills to enable them to successfully manage the transition to return to family, foster care or to live independently in the community. The post holder will ensure that the support plans are being adhered to by all multi-agency professionals to improve outcomes for young people.

Aspire Hub Practitioners will drive the action planning, interventions and case progress for young people through effective communication with a range of professionals working together to improve outcomes for young people.
You will support care leavers on the edge of care (including some post 18). You will work within a defined culture and intervention practise model and will adopt a solution focussed and restorative approach. Applicants must hold an NVQ 3 certificate in caring for children and young people, or be willing to work towards one.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
