Job description


Acute Inpatient Service
Sevenacres, the Isle of Wight’s psychiatric inpatient unit, consists of:
• Osborne Ward - a 16 bedded working age adult acute ward.
• Seagrove Ward - a 6 bedded psychiatric intensive care unit
• Afton Ward – originally a 10 bedded acute inpatient unit for functionally ill older adults, currently supporting a dementia pilot program; Home Treatment Team (HTT) and Dementia Outreach Team (DOT) undertake gatekeeping and discharge facilitation to all wards.
These wards provide inpatient facilities for patients who are severely ill and not able to be supported by the HTT service or DOT for patients with a diagnosis of dementia.
ECT
Electroconvulsive therapy (ECT) is a safe and highly effective treatment for a number of severe mental illnesses. ECT is presently provided at a dedicated clinic and takes place twice a week on Tuesdays and Fridays at Sevenacres building on the St Mary’s Hospital site.
The Mental Health service has its own pharmacists and pharmacy technician who take a lead in Mental Health treatments and are available for advice and support to all staff and patients as required.
Home Treatment Service
The Home Treatment Service offers intensive support to patients referred by the inpatient units (to facilitate discharge) or community teams (to provide an alternative to admission). These patients will have severe disorders and would otherwise have been admitted to an inpatient unit. The service operates within clear criteria for assessing whether any given patient is appropriate for home treatment. Following the home treatment phase, the patient will be transferred to the Community Mental Health Team or transferred to the inpatient unit should that become necessary. The gatekeeping role of the Home Treatment Service has been full adopted for access to inpatient beds, except for admissions under the Mental Health Act.
This service is not age based and works with all adults over the age of 18. However, there are several nurses within the team that specialise in working with older people. Psychiatrists can refer patients for intensive community nursing care during bouts of serious illness. Old Age Psychiatrists retain Consultant responsibility during this period of care.
Mental Health Liaison Service
Mental Health referrals from the Emergency Department, Medical Assessment Unit or the general hospital wards are dealt with across 24 hr period by the Mental Health Liaison Service, who assesses and organise further intervention or follow up as appropriate.
Community Mental Health Service:
Access to Intervention
The A2I offers urgent assessment of people and telephone advice for GPs, the acute hospital and other agencies between the hours of 8am to 10pm 7 days a week. They offer a range of brief time limited interventions to prevent urgent needs escalating, which may then require further interventions from secondary services.
Adult CMHT
The CMHT includes a multidisciplinary community Mental Health service, primary care psychological therapies, and early intervention in psychosis services. A transformation programme is currently redesigning our community Mental Health pathways in order to ensure they are more accessible and responsive to individual need.
Rehabilitation and Recovery Services
The Mental Health Recovery Service is a combined multi-disciplinary Team established in April 2021.
The team provides a spectrum of recovery orientated treatment and support to the population of the Isle of Wight, through Reablement, Intensive Rehabilitation and Assertive Outreach approaches. The team has clinical and managerial leadership which supports the provision of psychologically informed treatment approaches, in line with NICE guidance.
The team will work flexibly to deliver health and social care support to people in their own homes or supported accommodation. The Recovery team will provide an in-reach service to inpatient beds, working in line with Care Act compliant goals and outcomes. This support can then follow the person out into the community where they are able be supported by differing degrees of intervention according to level of need and complexity including an assertive outreach community team and the Reablement team.
Learning Disability Service
The Trust runs a comprehensive community service for people with learning disabilities and co-morbid psychiatric disorder consisting of a multidisciplinary team. An adult ADHD and AD assessment service is also provided.
Community CAMHS
The recently reconfigured Community CAMHS Team incorporates both Tier 2 (Primary Mental Health) and Tier 3 (Specialist CAMHS). The service is provided by a multidisciplinary team who work well together. The service includes colleagues from the Inreach/Outreach team who offer urgent, and crisis care for children and young people presenting in crisis both in hospital (through liaison with the acute hospital, paediatric and all age Mental Health liaison team) as well as in the community through offering a short period of intensive outreach work. There is a specialist eating disorders team for children and young people that works closely and collaboratively with the generic CAMHS team. The Community Specialist CAMHS Team enjoys excellent links with colleagues in other areas of Mental Health, social care, education, and voluntary sector.

Principle duties of the post
These posts involve working within one of the specialties outlined in the job description under the supervision of designated Clinical Supervisor.
Post holders will work closely with multi-disciplinary teams and provide medical support to the specialty they are involved in.
Clinical Responsibilities
To attend the wards and outpatients as required.
To participate in the relevant duty rota as detailed in the section out-of-hours cover arrangements.
Psychiatry:
• To undertake psychiatric assessments and reviews of patients referred to respective services, under indirect consultant supervision.
• To provide appropriate clinical cover in cases of emergency or urgent clinical need.
• To support a seamless service and continuity of care, especially with the community Mental Health services.
• To support a patient-centred service, encouraging social inclusion and respecting diversity.
• To work with other clinical staff in achieving high standards of assessment and care for all patients presenting to the service.
• To adopt the Care Programme Approach and support integrated care pathways.
Governance, Education and Teaching
• To contribute to the teaching programme on site.
• To attend regular teaching seminars held by the Consultants and other medical staff.
• To participate fully in Clinical Audit.
• To attend departmental meetings.
Professional
• In line with General Medical Council (GMC) and Good Medical Practice it is the responsibility of the post holder to ensure that all duties are carried out to the highest possible standard, and in accordance with current quality initiatives within the area of work.

• Understand duty to adhere to policies and procedures applicable to infection prevention and control.
• Comply with key clinical care policies and protocols for prevention and control of infection at all times; this includes compliance with Trust policies for hand hygiene, standards (universal) infection precautions and safe handling and disposal of sharps.
• All staff should be aware of the Trust’s Infection Control policies and other key clinical policies relevant to their work and how to access them.
• All staff will be expected to attend prevention and infection control training, teaching and updates (induction and mandatory teaching) as appropriate for their area of work, and be able to provide evidence of this at appraisal.
• You must adhere to and work within local and national safeguarding children legislation and policies including the Children Act 1989 and 2004, Working Together to Safeguard Children 2013, 4LSCB guidance and the IOW Safeguarding policy.
• You must adhere to and work within the local Multiagency safeguarding vulnerable adult’s policies and procedures.
THE PROVISIONAL TIMETABLE
• The provisional timetable will be agreed with the supervising consultant.
APPRAISAL AND REVALIDATION
The Trust has the required arrangements in place to ensure that all doctors have an annual appraisal with a trained appraiser and supports doctors going through the revalidation process.
Annual appraisal is currently undertaken by trained Educational Supervisors. Mr Stephen Parker is Medical Director and Responsible Officer for revalidation.

Commencement of Duties
The successful candidate will be required to take up the post within 3 months of the interview date unless a special agreement has been made between the appointee and the Trust. General Conditions of Appointment
Terms and Conditions of Service (TCS) are in accordance with the Doctors in Training – England contract 2016.
Annual leave on commencement is 27 days plus bank holidays increasing to 32 days after 5 years’ service plus bank holidays. The leave year is 1 April to 31 March. All authorised absence must be taken in accordance with leave protocols.
The post is pensionable within the NHS Pension Scheme (unless the appointee opts out or is ineligible to join).
The Isle of Wight requires the successful applicant to have and maintain full registration and a licence to practise with the General Medical Council.
Because of the nature of the work involved in this appointment, this post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offender's Act 1974 (exceptions) Order 1975. Applicants are therefore not entitled to withhold information about convictions for which for other purposes are “spent” under the provisions of the Act and in the event of employment; any failure to disclose such convictions could result in dismissal or disciplinary action by the Trust. Skills

Applicants have undertaken medical training in English.
