We are looking for an innovative, experienced Learning Disability Nurse, to join our Directorate as Lead Nurse for Learning Disabilities. In this role you will provide professional leadership for Learning Disability Nursing, effective clinical leadership for practitioners working with individuals with complex Learning Disability and specialist needs and will be pivotal in the development of services.
We believe that people with a Learning Disability have the right to the same opportunities as anyone else, living satisfying and valued lives and being treated with dignity and respect. They should expect to live in their own homes, develop and maintain positive relationships and get the support they need to be healthy, safe, and active members of society.
We are aware, a bigger focus is needed to ensure services are joined up, making it easier for the people with a Learning Disability to access help and support when and where they need it.
Its our aim to develop a new service model with the help of people with learning disabilities, their carers, health staff, providers, and commissioners. We believe that implementing a new model and giving people great power over the services they use, will result in a significantly reduced need for inpatient care and enable people to lead better lives.
As Lead Nurse for Learning Disabilities you will be pivotal in shaping, implementing and evaluating a new service model, alongside the Head of Service and Multi Disciplinary Team.
To provide compassionate, proactive and visible professional leadership within the Learning Disabilities Service, to ensure safe and high quality services for people with a Learning Disability.
To lead the development of Learning Disability Nursing to maximise its role in providing accessible, safe and effective evidence-based care and support to service users and carers, tackling health inequalities and promoting the rights of people with a Learning Disability.
To lead the review, improvement and on-going monitoring of Learning Disability clinical pathways and oversee the re-design of services and workforce models that support a continual and cyclical process of innovation and improvement.

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
