Job description

Key Dimensions: To oversee the reporting of medication incidents through the Trusts incident reporting To lead on the medicines Governance and safety audits program within the Trust. Use incident reports, clinical audits, local intelligence gathering, and other sources to identify trends, themes, and topics in the medication safety that occur in the organisation in line with the national safety strategy. To identify national Medicines Safety issues and their impact on UHP To lead on the review of Clinical Guidelines, Patient Group Directions, Standard Operating Procedures and risk assessments, engaging authors and specialist pharmacists to ensure all documents meet the required standards. Support the Associate Chief Pharmacist for Governance and Medicines Safety to ensure the outsourced pharmacy service is compliant with governance processes and standards (local and national). Take part in multidisciplinary Medicines Management walk arounds, as an expert for Pharmacy and MedicinesGovernance and safety. Support Associate Chief Pharmacist for Governance and Medicines Safety and assist with the in-depth investigation of specific medicines incidents as requested by senior pharmacy or medical colleagues or the Trust Serious Incident Review Support the incident management process for medicines at the Trust through reading and reviewing medicines incidents. Where appropriate liaising incident reporters/managers to conduct further investigation, if needed or to support the implementation of appropriate actions following a medication incident. To improve the incident reporting system, facilitating improvement in the frequency and quality of To establish a strategy for pharmacy safety and governance communications. Assist with preparing the quarterly Medicines Safety report and divisional . Support the Associate Chief Pharmacist for Governance (MSO) and Medicines Safety to provide Medicines Safety-related information in response to ad hoc requests from Trust committees, relevant Trust employees, clinical commissioning groups, or other external agencies as required. Present and discuss trends, themes, or topics in medication safety in relevant Trust forums, when asked to by the Associate Chief Pharmacist for Governance (MSO) and Medicines Safety. Work with the Associate Chief Pharmacist for Governance (MSO) and Medicines Safety and other Trust colleagues to contribute to the trust strategy for improving Medicines Safety. To be a member of and contribute to the agenda for the Trusts Medicines Safety Committee. To attend and contribute to the Trusts Drug and Therapeutics Committee. To contribute to medicine safety initiatives by engaging with national organisations and associations. Encourage the reporting of adverse drug reactions via the MHRA Yellow Card Reporting To write and review policies and procedures relevant to specific to Governance and Medicines Safety or clinical areas of interest Provide clinical pharmacy input at either ward or outpatient level to a defined cohort of patients (if appropriate) To act as a role model as a clinical pharmacist to pharmacists, technicians, pharmacy support staff and all other members of the multidisciplinary team. To be a lead and role model in your chosen area of clinical practice .
