Link Worker, Transitions Team, Adult Social Care

We look at providing support to enable young people to become as independent as possible in all areas of their life, which can include employment, social activities, personal care and independent living. As a Link Worker, you will be supporting young people on a one to one basis to achieve positive outcomes in all of these areas alongside the allocated social worker or assistant care manager. You will also be responsible for completing Care Act assessments for a small number of people on your caseload. The Link Workers attend school reviews for young people referred to the team in years 9, 10 and 11 as well as running the planning live events for the Transitions Team.
