Job description

To take responsibility for the overall day to day leadership and delivery of key workstreams, working with partners across West Yorkshire. Including management of staff and programme resources as appropriate To ensure the right level of rigour and discipline in relation to the workstreams, reporting progress and management of ris To ensure effective Public Health, clinical and analytical engagement into the work programme from across all places in the Partnership To lead workstreams (including wider networks across the partnership) in an effective way to ensure the programme is realising its objectives To work across the partnership to establish key working relationships which will enable successful delivery of the programme To provide consultant level Public Health expertise as required to inform collaborative partners and be the expert interface between the NHS Trusts and Public Health colleagues Work with partners to agree strategic objectives for the workstreams and align this with delivery and measurable objectives Determine how the collaborative and programme can add value in improving the health of the population through: addressing complex problems, sharing learning and best practice and identifying areas to work at scale Act as an advocate for the principles of population health including; reducing health inequalities, a whole-system approach to prevention, addressing the wider determinants of health underpinned by an intelligence led approach of population health management Maintain contact with the Senior Responsible Officer for the programme, MHLDA programme director, wider Public Health colleagues and the programme director for Improving Population Health Make the most of the expertise we have in WY to Improve Population Health to support the work of the programme and collaborativ Lead specific workstreams within the MHLDA programme; including line management for any additional project staff, developing and maintaining project/programme plans and setting team objectives that are aligned with the ambitions of the programme Provide MHLDA leadership into the Improving Population Health Programme Board and associated Programme Networks and Task and Finish Groups Support the communicate of the vision for Improving Population Health across the partnership To support the use of Population Health Management tools and techniques across the collaborative and place-based MHLDA partners, including primary care and VCSE providers. To include linked person level data, population segmentation, risk stratification, impactibility modelling and opportunity analyses Work within a complex system environment to support partners to identify priority areas for action.
