1.Primary Duties Manage your own caseload of patients whilst working as part of the multi-disciplinary team . Attend MDT meetings. Communicate highly sensitive and emotive condition related information to patients and relatives e.g terminal illness, bereavement. Assess new patients in the clinical area utilising the holistic needs assessment tool to identify physical, psychological emotional, spiritual and cultural needs and developing a plan as needed. Act as patient advocate when relevant and be aware of all treatment options in order to enable patients to make an informed choice, negotiating with medical teams to agree a pathway of care and treatment. Provide continuity of care throughout the patient journey, liaising across specialist services where required. Contribute to the development of specialised programmes of care and provide written and oral information to patients and relatives. Provide clinical advice and support to healthcare professional within and across organizational boundaries. Act as a role model. Initiate changes in management of care. Provide support for distressed patients/carers and bereaved relatives, identifying patients at risk and referring to appropriate agencies. Maintain and update evidence based knowledge and skill. Undertake complex assessments and review of patients and carers. Contribute to the development of specialist protocols and policies for specialist area care. Be a member of relevant professional association and attend regional/national meetings and conferences General Responsibilities Assist in developing in house specialist educational and training programmes. Maintain the prevention and control of infection and fully comply with all current Trust Infection Control policies and procedures. Take responsibility for any records that they create or use in the course of their duties, in line with the Public Records Act and be aware that any records created by an employee of the NHS are public records and may be subject to both legal and professional obligations.
