Job description


An exciting opportunity has arisen for Nursing associates to join Cancer Clinical Nurse Specialist Teams to help support patients and deliver the Personalised Care agenda to our patients for a 24 month fixed term basis.
Each post is Macmillan funded for 24 months and will work in one of the following cancer CNS teams Colorectal surgery and Gynaecology specialty. The post will either be at QMC site, City Hospital site or a combination of both depending on the specialty.
Each post will be embedded within Cancer Clinical Nurse Specialist Teams and work closely with each MDT to offer and support patients in the completion of Holistic Needs Assessments and the development of a support plan, as well as to support in the implementation of treatment summaries and a sustained personalised stratified follow up pathway.
We are seeking enthusiastic and highly motivated individuals with excellent communication, interpersonal and organisational skills to work in this innovative role, demonstrating how the unique role of the Nursing Associate can support both our patients and our Clinical teams.

The Macmillan Nursing Associate role will work as an integral part of Cancer Nurse Specialist Teams to support patients living with Cancer and assist with delivering the personalised care offer for all patients in line with their scope of practice.
The overriding purpose is to support the provision of the highest quality patient/client care through personal actions and continuous improvement.
The post holder will support the Registered Nurse in ensuring the delivery of high quality personalised patient/client care upholding dignity, delivered with kindness and compassion.
Support the Cancer Nurse Specialist Team in delivering the core principles of the Personalised Care in Cancer services, specifically the ‘Macmillan Right by You Principles’.
Provide and monitor supportive care for a designated group of patients/clients during a care episode within Cancer care services.
Actively contribute to an effective learning environment and support others in their learning as appropriate.

To contribute to assessment, planning, implementing and evaluation of patients/service users care and management, ensuring provision of patient/client centred care in Cancer.
Supports the cancer nurse specialist team by having appropriate regular supported conversations with the person living with cancer, assisting in distributing Holistic Needs Assessment tools for patients to complete. Supports the CNS in providing relevant information and advice and signposting the patient to other Macmillan resources and services supporting them to self-manage.
Following a holistic needs assessment, assists the CNS to coordinate care with the person living with cancer so they can access the right help at the right time, providing a seamless experience of care. Checks that the person is able to self-manage and provides a flexible and personalised response to any changes in need.
Navigates the complex health and social care system, both during and following completion of cancer treatment, by building relationships with professionals and facilitating access to these services including supporting the CNS to generate referrals on the person’s behalf where necessary.
Supports the CNS to provide practical and emotional support to the person living with cancer and their families. Recognises information needs may change over time and works with the person to ensure they have all the information and support they need at all stages of their cancer journey.
Supports the CNS to document and monitor all aspects of care coordination and service delivery, supporting data collection on ‘Macmillan Right by You’ acute Performance Metrics, and how people living with cancer use the service, including care, support and follow up services they access.
To demonstrate good knowledge of care and treatment in order to undertake non-complex nursing assessment/reassessment in various settings under the direction/ supervision of senior staff.
To work alongside partners in the community, hospitals and care homes to gather information for the purposes of completing assessments and review.
Work with other team members to provide support and advice to patients and carers in relation to nursing needs.
To maintain accurate, clear, concise and contemporaneous records and communicate information to members of the team as necessary.
To liaise with external agencies with regards to the development and implementation of care packages.
To contribute to the development, delivery and evaluation of service improvements and quality initiatives.
Able and willing to travel in order to carry out planned interventions at various location under appropriate delegation/supervision.
To be able to work independently within Nursing Associate scope of practice, prioritising own work as required, exercising initiative and judgement.
Communicate with colleagues and stakeholders around issues within own area of work, which have a level of complexity such as dealing with business sensitive information, or requiring co-operation from staff outside of department such as patients, carers, families and where persuasive skills will be required.
Guided by organisational and professional policies but has the freedom to establish the way in which these should be interpreted.
Ensure own mandatory training is kept up to date, including clinical supervision.
