JOB DESCRIPTION Post Nurse Associate Salary Grade Accountable to Nursing Services Manager Reporting to GP Practice Lead Nurse PURPOSE OF THE ROLE The post holder will have completed their Nursing Associate training and be registered with the NMC and work independently under the leadership of registered nurses, working within the sphere of nursing and care, and within all aspects of the nursing process. Nursing associates are equipped with the clinical and theoretical knowledge, understanding, skills, attitudes, and behaviours relevant to employment as a Nursing Associate and will work to a nationally recognised code of conduct (Nursing and Midwifery Council 2015). The role will assist the registered staff to deliver: To provide a range of nursing care duties to patients registered with practice, under the supervision and direction of a registered nurse. To make the best use of clinical skills within the practice in order to promote well-being and maximise health gain for patients To compliment the role of the registered nurse and other Health Professionals as an integral part and key member of the care team To provide a safe, high quality and accessible treatment room service to the practice population MAIN DUTIES AND RESPONSIBILITIES - (This is not an exhaustive list) Work within the GP practice, supporting the practice nurses, and practice team reporting back to the registered nurse on a regular basis. Assessment and planning of delegated care and contribute to the delivery of integrated care. Provide nursing care to patients who attend the GP practice and in their own home as directed by the lead nurse. Work as part of the GP Practices MDT to provide continuing care and monitor care, under direct or indirect supervision. Work with the GP Practices MDT to ensure delivery of Nursing Associate duties complement existing workforce. Provide support and supervision to training Nursing Associates, healthcare assistants, apprentices, and those on learning assignments / placements as required. Support registered nurses to enable them to be able to focus on the more complex clinical care. Perform and record clinical observations such as blood pressure, temperature check, respirations, and pulse. After undertaking additional training, and run clinics that provide flu vaccinations, ECGs, phlebotomy, urinalysis and venepuncture, and other relevant clinical tasks as required by the GP practice, in line with the competencies of the NA role and within the practice policy. Promote health and well-being and self-care management to all patients, for example undertaking the NHS health check; as well as the monitoring of long-term conditions including escalation of interventions when necessary. Care for individuals with dementia, mental health conditions, and learning disabilities; utilising, supporting and referring to PCN and core community services such as social prescribers, as appropriate. Advise patients on general healthcare and promote self-management and self-care where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services. Maintain accurate and contemporaneous patient health records; and preserve confidentiality at all times. Escalate any concerns to the registered nurse in charge. Administering of Medication in accordance with regulatory guidelines for Nursing Associates. Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment when appropriate. Be able to perform cardio-pulmonary resuscitation according to current UK guidelines. Pro-actively prevent and protect against disease and ill health & Act in the best interests of service users, carers, families, and communities cared for. Use knowledge and experience to make evidence-based decisions and solve problems Recognise and work within the limits of your competence. Engage in reflective practice including management of self and reflection on own reactions, asking questions and reflecting on answers given. Assist patients to identify their own health needs. Management of Chronic Disease: To develop knowledge base and support the management of Chronic Disease patients within the primary care setting. Maintain accurate records in line with practice policy and NMC Guidelines using appropriate computer software. Responsible for the maintenance of equipment and stock control: including refrigeration, vaccine ordering, single use equipment, general treatment room equipment and stock and emergency rugs and equipment. Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service. Be responsible for own actions and act professionally at all times. Identify personal development and training needs in conjunction with the Nursing Services Manager. Participate in an annual individual performance review, including taking responsibility for maintaining a record of own personal and or professional development. Participate fully in practice audit and quality development programmes. Participate in formal tutorials and informal training of student nurses and GP trainees in the practice. Checking compliance with and adherence to appropriate treatments using a holistic patient cantered approach. Co-operate and participate as required in any research projects across the GP Practices. Any other duties considered relevant to the post. CULTURAL Learning & Development Ensure continuing education, training and development is undertaken to meet role specific and governance guidelines for Continuing Professional Development. Continue to develop soft skills and emotional intelligence in line with the requirements and culture and to uphold the values and culture of Belvoir Health Group.
