Highland homebased with travel
23 hours per week / Fixed Term 12 months / Happy to talk flexible working
£30,199.03 pro rata (£19,845.08) per annum
Job purpose:
To promote the Carers (Scotland) Act 2016 and Carers rights including the benefits of an Adult Carer Support Plan (ACSP) in supporting Carers to maintain their own well-being and day to day life.
Carer Support
• To provide person centred support to Carers, who care for a person with a Mental Health illness or is experiencing Mental Health problems, to assist them to complete an Adult Carers Support Plan, which will include establishing their identified personal outcomes and identified needs, which are realistic and achievable via outcome-based conversations and considering and creating future and emergency plans, as well as accessing appropriate services which support them in their caring role
• Source and provide information for Carers relevant to their caring situation and provide links to other resources and support services
• Work with Carers to identify training and learning needs, and direct Carers to appropriate courses and learning opportunities
• Work with Carers to identify the emotional impact of caring or bereavement and direct them to appropriate support, including Carer Support Groups and Counselling Services
• Work with Carers to identify their health needs and support them to take appropriate health promoting activities
• Advise Carers on benefit entitlements and assist by introducing and referring to Income Maximisation services
• To regularly review the progress toward the outcomes identified by Carers as part of the Adult Carer Support Plan process
• Facilitate a social group on a monthly basis for Carers, who care for a person with a Mental Health illness or is experiencing Mental Health problems
Partnership Working
• Assist the Adult Carer Service Manager (ACSM) in partnership working with all members of the Multi-disciplinary Health and Social Care Planning Team(s)
• To promote the role and needs of Carers to professionals with a particular focus on Social Workers, Multidisciplinary Teams and hospitals
• To actively promote Connecting Carers to Carers, statutory providers, voluntary organisations, and any other appropriate agencies
• To work in partnership with Mental Health services such as New Craigs, Children and Adolescent Mental Health Services (CAMHS), Change Mental Health and others
• To respond to enquires when requested and represent the service at locally based community events
• To actively work in partnership with Social Workers, GP Practices, Community Health teams, statutory agencies and voluntary organisations to identify and support Carers
Outcome measuring and evaluation of support
• To monitor the effectiveness of the service intervention by reviewing the outcomes identified via the Adult Carer Support Plan
• To actively seek feedback from Carers and professional partners
• To use Connecting Carers monitoring and evaluation systems including the Carers database and Outcome Star to evidence outcomes for Carers
• To maintain a record of the outcomes identified by Carers as part of the Adult Carer Support Plan process
General duties
• Comply with the Carers Centre policies and procedures such as confidentiality policy
• Seek funding opportunities for both Carers and the organisation
• Take responsibility for Continuing Professional Development by taking part in relevant training and produce work plans
• Provide Carer Support Line and Carer Chat cover during periods of annual leave/sickness
• Attend regular Support and Supervision and caseload management meetings
• Participate in staff meetings
• Undertake any other tasks or duties necessary to achieve the goals of the organisation under the direction of the ACSM
