Job description


We are looking to appoint an enthusiastic, motivated, flexible, hardworking, positive thinking and experienced QIS neonatal nurse to assist in the delivery of a seamless transition from hospital to home for babies and their families within NICU, the Transitional care and outreach service.
The Neonatal Outreach service is provided seven days a week, with team members liaising with both hospital based and community multi-disciplinary teams, whilst working to avoid or reduce mother and baby separation through earlier discharge and prevention of readmission to the paediatric ward.
Continued support of babies and families post discharge from the Neonatal Intensive Care Unit
Escalation or signposting to other health care professionals
Provide strong leadership and direction to the team, monitoring and maintaining standards of care and professional practice to support day to day efficient running of the Neonatal Outreach service.
Act as a strong role model and provide development, clinical supervision and advice for other staff and students to support them in reaching their full potential. Actively engage in research and development opportunities for the service.
Assist the Team Leader in promoting a seamless outreach service and transition of the baby’s care from the hospital setting to home, providing a seven day service to families.
A fantastic opportunity has arisen to join our excellent neonatal service here at Lancashire Teaching Hospitals NHS Trust (LTHTR). Our Neonatal Intensive Care Unit is one of two Level 3 Units within the Lancashire and South Cumbria Neonatal Network. We take pride in providing the perfect environment in which our tiny neonates can grow, thrive and develop while supporting parents through this incredibly challenging experience. The Neonatal services comprise of a 28 cot NICU, a Neonatal Outreach Team and a Transitional Care Unit.
Here at LTHTR we have a dedicated and dynamic multidisciplinary team who maintain the highest quality and standards of care to our neonatal population, the successful applicant will join our dedicated team which includes Neonatologists, Advanced Neonatal Nurse Practitioners, Neonatal Nurses, Neonatal Outreach Service and a dedicated Education Team who provide clinical training and support within the neonatal unit. In addition we also work closely with a highly motivated team of allied health professionals and midwifery colleagues to achieve the best outcomes for babies and provide excellent family centred and psychosocial care for our patients and families. Here at LTHTR we focus our care and service delivery with care and compassion at the heart of all we do. The neonatal service is strongly focused on the Family Integrated Care agenda and supporting early transfer home of infants.
Support the Team Leader and Matron in
ensuring the provision of excellent care with
compassion for the babies and families who are
cared for by the outreach and transitional care
team. Ensure a high standard of nursing is delivered
by self and others based on assessment,
planning, implementation and evaluation of care
to meet babies and family needs in accordance
with agreed local and National standards and
evidence based guidelines.
Assist and support the Team Leader in the safe,
effective management of the team including
management of personnel, facilities and the
associated systems and processes
Act as a role model, presenting a positive image of
self, team and the organisation
Be responsible for the delivery and co-ordination of
care, through appropriate professional practice,
delegation and supervision of duties carried out by
junior staff
Set, monitor and maintain standards of care to
improve the patient experience and outcome of
care.
Proactively manage risk and maintain patient
safety, conforming to health, safety and security
legislation, policies, procedures and guidelines
Communicate all relevant information to the
multidisciplinary team internally and with external
organisations as required, to ensure seamless care
