We are looking to recruit a motivated and forward thinking Sexual Health Outreach Nurse to join the integrated Sexual Health team at Bolton Foundation Trust. A flexible approach which meets the changing needs of the service is essential.
The post holder will work in partnership with all members of the multidisciplinary team within the community settings of the Bolton borough. The post holder will actively contribute to clinical effectiveness initiatives including risk management, audit and guideline development.
The service provides an integrated sexual, reproductive and HIV service as well as specialist clinics. It operates on a Hub and spoke model with the main hub located at Crompton Health Centre. We are seeking someone passionate and knowledgeable about Sexual Health and wellbeing who is flexible and enjoys working with a diverse patient group.

The primary responsibility of the post holder is to assess, plan, implement and evaluate nursing care for patients, and to develop and promote an integrated service that encompasses all aspects of Sexual Health and contraceptive provision within sexual and reproductive health across the Bolton population.
Shift patterns will work to six days (once resumed following the suspension of Saturday clinics throughout covid) with the post holder working across all hubs clinics.

The Trust also has a tertiary level Neonatal Intensive Care Unit, enabling 1 in 4 women across Greater Manchester to receive in-utero transfer and birth at extreme premature gestation at Bolton.
