AddressCornerstone Medical Practice is looking for a Practice Nurse (15-20 hours a week) to join our established and friendly multi-disciplinary team.
We are a small but growing GP practice based in Salford seeking to appoint someone with a caring nature and proactive professional manner to provide a high standard of nursing care for our 4000 patients. This is a fantastic opportunity for a Practice Nurse with good communication skills and a commitment to working as part of a strong and effective clinical team.
Ideally the successful applicant will qualified to undertake chronic disease management (diabetes, and/or COPD and Asthma), travel health, women's health and general treatment room tasks. There will be flexibility for the right candidate
Main Duties of the job:
- Long term condition reviews & management, encouraging Patient self-management in Diabetes, Asthma, COPD, CHD, CVA, Dementia and Learning disabilities
- Cervical smears and promoting health
- Contraception reviews
- Routine immunisations including childhood vaccinations under PGDs
- Urinalysis
- Venepuncture
- Smoking cessation
- Visit patients where appropriate in their own home.
- Main Administrative Duties
- Blood pressure management/ hypertension management
- Requesting basic pathology test ex: urine culture, swabs
- Following agreed clinical protocols with referral to Nurse Manager or GPs as appropriate
Job Types: Part-time, Permanent
Salary: £19.00-£20.00 per hour
