Ward 2B- 17 patients
This post is for a Nursing Associate or Assistant Practitioner who will lead a team to deliver outstanding person-centred care along with a motivated MDT team.
To provide safe, personal and effective care and support to patients, families and service users. To support the registered nurse, contributing to ongoing assessment, care delivery planning and evaluation for patients. To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.
Administer medications if competent to do so
Assist with care planning and delivery of care
Admission and discharge support
Support patient rehabilitation
Complete documentation to a high standard
Actively involved in best practise and supporting any improvements
Clifton hospital offers enhanced clinical rehabilitation and social care rehabilitation pathways for people who need a short period of intensive support in a 24 hour setting.
Clifton hospital accepts patients on step down pathways transferred from acute based care or step up pathways for patients requiring an escalation of need from home and to prevent admission to hospital.
Social and health care professionals work in an integrated way to provide individually tailored support programmes to help people maximise their independence.
The team is made up of nurses, occupational therapists, physiotherapists, medics, and Social Workers who all work in partnership to identify goals to promote independence

• Apply and promote safe and effective practice that places the individual and/or family/carer at the centre of care, in a manner that promotes individual wellbeing and self-care.
• Display a personal commitment to professional standards and ethical practice, operating within national and local ethical, legal and governance requirements.
Care Delivery
• Deliver holistic, person centred nursing care
• Actively engage with individuals, their families and/or carers and contribute to care planning, by establishing their needs, wishes, preferences and choices and incorporating these into care planning.
