Main duties of the job:
This post is full time but applicants who wish to work part time or flexibly will also be considered.
The service runs 8am to 6pm Monday to Friday and 9am to 5pm at weekends. The post holder will be expected to contribute to the weekend rota.
The service covers both East and North Hertfordshire and the post holder may be expected at times to cover across all localities. Travel costs will be covered in line with our Travel and Expenses policy.
You must hold a full valid driving licence and have access to a car to use regularly for business purposes are essential (unless you have a disability as defined by the Equality Act 2010).

Detailed job description and main responsibilities:
To be part of an integrated Health and Social Care Team. To provide clinical leadership and support to the multidisciplinary team in providing care to avoid inappropriate admissions to hospital and support hospital discharge.
To provide assessments, and treatment, where required for vulnerable patients with potentially complex needs. To support patient-led goal working within the community.
