Contract Type: 3 month initial Contract - Full time or 4 days a week

Salary: £36 an hour

About the Role

We have an exciting opportunity available within the Hampshire County Council Adults' Health & Care Reablement Service. We are seeking highly skilled, enthusiastic and self-motivated Occupational Therapists to join our service in Hampshire.

You will be working primarily in Fareham but some countywide work is likely

What is the day-to-day of the role:

• Work collaboratively with a multi-disciplinary team in order to identify and respond to the needs of people with disabilities, enabling them to live as independently as possible within their own homes
• To coordinate a specific programme of intervention in the community if needed and primarily working in our bed based re-ablement service.
• Assessing users to participate and maintain maximum personal independence
• Assessing carers' needs and abilities to support people with disabilities
• Maintaining case records and reports
• Providing information to duty and other professionals / teams
• Advising people on rights and benefits
• Sign-posting people to correct location to obtain service required
• Advising on appropriate choice of special equipment, adaptation and housing needs
• Read and adapt plans for adaptations and advise on the needs of disabled people with a view to adapting their home to maintain independence

Required Skills and Qualifications

• 2 years post qualifying experience
• Registered with HCPC
