Band 5Occupational Therapist - Newham, London
The Extended Primary Care Team in Newham is looking for a B5 Occupational Therapist. You will be joining a multidisciplinary team of nurses, therapists, nursing associates and healthcare assistants. You will have support from a wide range of professionals who work with us, including Occupational Therapy and Physiotherapy clinical leads, senior therapists, senior nurses, safeguarding and psychology professionals.
You will work closely with B6 and B7 Occupational Therapists, carrying out joint visits with physiotherapy colleagues when appropriate. Your professional focus will be rehabilitation in the community for patients who have experienced a deterioration of their physical health, but you will work closely with Occupational Therapists in other teams when needed, including the in-house rapid response and fast falls team and social services.
You will have monthly clinical supervision and a yearly appraisal to identify your on-going professional development needs. You will be provided with an iPad or a laptop so that you have access to all clinical information for your patients when you are in the community. In conjunction with other members of the team and with supervision, contribute to the provision of a comprehensive client-centred occupational therapy service to patients. Patients are referred primarily due to a physical disability and/or illness, are aged 18 and over and are resident in the community. This includes patients who may present with physical, cognitive, psycho-social and housing needs as a result of a wide range of medical conditions.
To manage a defined caseload in a community setting. To evidence problem solving and clinical reasoning skills in line with evidence based/ client centred principles. To assess, plan, implement and evaluate interventions
To be an active member of the multi-disciplinary team, providing integrated, goal-orientated interventions
To develop a wide range of occupational therapy skills working with patients with a physical disability
To contribute to the planning, development and evaluation of the service
The Extended Primary Care Service enables the provision of multi-disciplinary care and support to vulnerable adults in a range of community settings. Patients are referred primarily due to a physical disability and have complex medical and social needs. Nursing, therapy and support staff work liaise closely with local GPs to provide the majority of community health services for adults living at home
Occupational therapists work as part of the multi-disciplinary team, providing patient-centred occupational therapy interventions, to enable patients to maximise their independence in daily activities in their own homes.
