The Community Specialist Palliative Care Team provides holistic, highly specialist rehabilitation and complex symptom management to improve the quality of life for patients and their carers with a complex Palliative Care diagnosis. This post holder will participate in the development of a clinical specialist Occupational Therapy service as an integral part of the Community Specialist Palliative Care Team. They, in conjunction with senior colleagues, will assist in the review, development, planning and co-ordination of the service as deemed necessary to meet objectives. The post holder will work as part of a specialist multi-professional team, across a variety of settings, including working with patients and their carers in a community setting where patients require the management of complex Palliative Care symptoms. The post holder will be required to provide clinical leadership in the area of Specialist Palliative Care to support the community specialist, multi-disciplinary Palliative Care team and Occupational Therapy service in managing the full range of functional, positioning, personal care, psychological needs and social needs that may be experienced by patients with Palliative Care needs. They will be required to liaise with colleagues (multi-disciplinary) from the acute, community, intermediate care sector, and voluntary sectors. They will take a lead role in the advanced assessment and treatment of patients within this speciality who have complex Palliative Care needs, determine clinical diagnosis, deliver occupational therapy assessment and intervention indicated and maintain records as an autonomous practitioner..

A minimum of 5 years post qualifying experience as an Occupational Therapist at least 3 years of which must be at Band 6 or equivalent level in an Adult Community/Community Learning Disability/Community Rehabilitation Occupational Therapy setting , Have completed Foundation level communication skills training e.g. Sage and Thyme Foundation Communication Skills TrainingAttended Foundation level palliative care training e.g. provided by Local Trust /Regional training
