- To develop and deliver specialist Occupational Therapy for people with long term conditions and palliative care needs within the community and hospice.
- To work within the Doncaster Community Therapy Service to deliver the Home First model to people in the Doncaster area, to prevent hospital admissions and accelerate hospital discharges.
- To assess individual patient’s needs, set and implement treatment plans and long term condition management for patients linking with primary, secondary and tertiary care, using a coordinated approach.
- To contribute to the development of the Occupational Therapy role within the Occupational Therapy Service by working across boundaries and interagency working.
- As part of a team, initiate research based practice and monitor the quality of the service being provided.
