About the Manchester Outreach team

In Manchester, our services extend to care leavers, family placements, Unaccompanied Asylum-Seeking Children (UASC), mother and baby placements, as well as adults and NHS referrals. The caseload within our team is diverse, ranging from caring for a 17-year-old to supporting individuals as old as 60. Our clients present a spectrum of needs, including mental health challenges like psychosis or autism, and some have been in the care system throughout their lives.

We are committed to providing comprehensive support, encompassing independent living skills development, assistance in accessing education or employment opportunities, medication reminders, support with benefits applications, accompanying individuals to appointments, active participation in meetings, and offering crucial emotional support. Our team is dedicated to fostering the well-being and progression of individuals across various age groups and needs within the Manchester community.

Due to the areas this role covers a car driver with a vehicle is essential.

About you

The ideal candidate will be passionate about this sector, looking to make a difference with our service users and will be looking for a challenging but rewarding role.

Essential

  • Previous experience supporting (not caring) vulnerable people, families or young people to gain independence.
  • Travel is essential so must hold a full driving licence and have access to a vehicle insured for business use.
  • Must be prepared to travel in the Manchester area
  • Thorough knowledge of safeguarding vulnerable adults and children
  • Have or be willing to work towards an NVQ Level 3 Health and Social Care qualification.
  • Computer literate
  • Good interpersonal skills

Key Duties:

· Managing a caseload of vulnerable Service Users, helping to build independent living skills

· Liaising with relevant professionals such as Social Workers, Healthcare practitioners and the Police

· Writing detailed reports which assess a service user's independent living skills

· Completely weekly inspections of Service User's properties

· Support with accessing and maintaining education/employment

· Promote physical, mental and emotional well-being, de-escalating challenging situations when they arise

· Signposting to appropriate services as required, including mental health, drug rehabilitation services, counselling and education
