As the Systems Transformation Lead the post holder will work alongside the ICBs PEoLC Programme team to improve the outcomes for patients and the sustainability of PEoLC and: Develop an understanding of the local assets, needs, and inequalities, to support the development of PEOLC strategic plans using the Ambitions Self-Assessment Framework. Raise the profile of PEOLC across the local policy and broader strategic agendas. Support the statutory core responsibility of ICB's to ensure that system wide accessible high quality, safe palliative and End of Life Care, tailored to the needs of the individual (Health and Care act 2022) is available. Provide an innovative, ambitious, and solution-focussed approach to unblocking barriers and leading the transformation of services to meet the individual needs of patients and address inequalities in care provision. Support the strategic direction of new and widened collaborative opportunities to transform and develop of sustainable PEOLC services. Develop and support local, regional and national networks to share PEoLC best practice and learn from others' successes. Empower people with lived experience and local subject matter experts to co-produce local services for local people. Assist in gaining a greater insight into present and future PEoLC need and to ensure alignment with the Partnership teams and their priorities.
