Job description


The Patient Safety Co-ordinator is a valued member of the Women’s & Children’s Patient Safety Team.
This role will be responsible for supporting the Quality & Safety Manager in the improvement of Patient Safety in all aspects of clinical practice and service delivery.
The post holder will be responsible for implementing all requirements in relation to incident management acting as a case manager/lead investigator and ensuring performance and quality standards are met.
There are two full time posts available.
Provide advice and support to the Clinical Management Group (CMGs) and others on all issues relating to Patient Safety (e.g. incidents and complaints).
Support the Quality & Safety Manager by reviewing Datix-web to ensure all clinical incidents are reviewed and graded appropriately and within the required time frames.
Support, advise and as required engage with the CMG to ensure compliance with the requirement to inform patients/relatives (Being Open/Duty of Candour).
Lead investigations, (wherever they originate) ensuring the use of Patient Safety Investigation tools and write findings/report identifying achievable actions that transfers in to individual and organisational learning.
Support the referral of relevant Patient Safety incidents to external organisations, including collation and provision of relevant documents to these organisations, liaising within and outside of the Trust and ensuring that robust action plans are developed in response to reports received.
Communicate with patients and their families in a sensitive manner to advise about Patient Safety incidents.
Undertake Rapid Reviews of clinical incidents that meet the threshold for this and ensure that appropriate levels of scrutiny are applied and that escalations of concerns are progressed if indicated.

Monitor themes, trends and clusters within case load and inform the Quality & Safety Manager, Clinical Director, Deputy Head of Nursing, Head of Nursing Deputy Head of Midwifery, Head of Midwifery and Head of Operations communicating issues of concern.
Support the wider Patient Safety Team with the analysis of incidents, identifying actions.
Develop and maintain effective working processes and relationships to ensure performance and quality standards are met.
Discuss sensitive and contentious information with staff of all grades and disciplines maintaining confidentiality and integrity at all times.
Ensure all communication with patients, relatives and carers are open, honest and that investigations are transparent and independent.
To ensure responses and reports are factual, well written and clearly identifying learning and actions.
Support the Quality & Safety Manager in ensuring staff have the knowledge and skills to improve Patient Safety through training, teaching and by example.
Undertake personal development, education and training in relation to Patient Safety to ensure you can meet the continued requirements of the job role.
Provide direct line management to the Band 2, Band 3 and Band 4 members of the team.
To process routine enquiries and correspondence in a timely and accurate manner, including drafting of letters, and drafting of incident report templates under the guidance of the Quality & Safety Manager.
Note: - This job description indicates the main functions and responsibilities of the post, however the duties are not an exhaustive. The post holder will be required to work flexibly within the Patient Safety Team and undertake other duties from time to time as we may reasonably require.
