We have a new exciting opportunity for a Patient Safety Incident Response Framework (PSIRF) Lead to join our Governance and Quality team. You will work across all Everyturn services to implement the Patient Safety Incident Response Framework (PSIRF) ensuring a consistent and thorough approach to Incident Responses and learning and also act as the key point of contact for matters related to PSIRF and Patient Safety and provide advice, support and mentorship. You will create, strengthen and promote an organisational culture of Patient Safety and clinical effectiveness, providing a visible presence that focuses on a just and learning culture, high quality care standards, and preventing avoidable harm by analysing and interpreting trends, lessons learnt and best evidence to support the development of an organisational wide positive safety culture. Key duties and responsibilities: To lead on the delivery of the organisations Patient Safety Incident Response Policy and Patient Safety Incident Response Plan, ensuring compliance with legislation, national standards, and regulatory frameworks. Lead on a range of Incident Responses including Patient Safety Incident Investigations, After Action Reviews, Case Note Reviews, thematic reviews, and safety improvement plans. Lead and facilitate after action reviews with staff to identify, agree and share learning for clinical services. This will involve using persuasion, negotiation and empathy on often sensitive topics relating to patients clinical safety. Lead on external briefing meetings or outcomes of Patient Safety investigations to patients or their representatives as required, requiring empathy and candour. This may involve facilitating discussions with patients and clinical staff on safety issues regarding care. Provide assurance through spot checks and audit on the effectiveness of the incident framework for the organisation (i.e. random sampling and checks on incident data and investigation decisions). About you Clinical qualification e.g. Registered Nurse, Psychologist, CBT Therapist, occupational therapist, PWP, Social Worker. Level 2 in Safety Investigation Experience of working in a clinical governance, Patient Safety or quality improvement related area within a healthcare setting. Experience of using tools and measures for reporting compliance activity. Experience of planning over short- medium- and long-term timeframes and adjusting plans accordingly. Experience in producing reports and presentations.
