PCN Care Co-ordinator

Spalding PCN presents an exciting opportunity for a Care Co-ordinator to join our innovative team, contributing to the reshaping of Primary Care across our vibrant community. The role will be based across our two surgeries within our Primary Care Network. Applications will be considered on a full or part-time basis.

Spalding PCN comprises of approximately 44,000 patients across two GP practices in the Spalding area. Our dynamic teams are committed to individual development, fostering flourishing roles to enhance patient access to services. Role Overview:

The successful candidate will support the PCN leadership team and Practice clinicians in coordinating key activities, ensuring timely, efficient, and patient-centred health and care planning. Responsibilities include overseeing the patient's journey through primary care, supporting digital initiatives, conducting health reviews, and contributing to effective care planning. This pivotal role involves close collaboration with the multidisciplinary team (MDT) within the practice and PCN-wide to ensure optimal patient care and service delivery.

Key Responsibilities:

  • Coordinate all key activities related to access to services, advice, and information.
  • Ensure timely and efficient health and care planning.
  • Collaborate closely with the MDT within the practice and across the PCN.

Requirements:

  • Excellent administration and organizational skills.
  • Effective time management and attention to detail.
  • Experience in care planning or project management (desirable but not essential).
  • Local travel between practice sites and care homes may be required.

Candidate Profile:

  • Skilled Care Coordinator with person-centered planning experience.
  • Local knowledge of key support services for the patient population.
  • Experience in healthcare or the voluntary/community sector supporting vulnerable groups.
  • Diploma Level 2 in Health and Social Care or equivalent qualification (preferred).
  • Strong listening and communication skills.
  • Familiarity with local resources and services.
  • Motivated by helping people with care and empathy.

Administrative Skills:

  • Proficiency in Microsoft systems (Word, Excel, Outlook, PowerPoint).
