We have an exciting opportunity for an enthusiastic Clinical Pharmacist to join our dynamic team. This permanent position is well suited to an individual who is enthusiastic about providing high quality support to primary care.

We are seeking a dedicated Clinical Pharmacist to join our primary care team. The successful candidate will be integral in ensuring optimal medication management, providing pharmaceutical care, and contributing to the overall health of our patients. This role demands a professional who is adept at balancing patient interaction with detailed pharmaceutical knowledge and compliance with medical protocols.

Based within Medway Peninsula PCN, you will work closely with GP Practices and Care Homes within the primary care network. GP Practices include Highparks Medical Practice and The Elms Medical Practice, Kent.

You will be joining forces with our established team of clinicians within the PCN, this includes Physician Associates, First Contact Physiotherapists, Paramedics, and Social Prescribers.

Although your working days and hours will be standard, your work patterns are likely to vary when providing clinics in GP practices seeing patients with a variety of conditions. Your allocation of patients will be according to your clinical skill set/competencies and following a triage process to ensure patients are allocated to the appropriate clinician.

Main duties of the job

Key responsibilities:

  • Conduct medication reviews and health consultations with patients, identifying potential drug-related problems.
  • Work with complex polypharmacy patients, implementing prescribing decisions and requesting appropriate tests.
  • Provide expert advice on drug therapies, optimizing medication regimens for safety and efficacy.
  • Reconcile medicines following hospital discharge and work with patients and the community pharmacists to ensure that the patients receive their medication post discharge.
  • Care home ward rounds to carry out medication reviews and de-prescribe as appropriate.
  • Collaborate with general practitioners and other healthcare professionals to develop comprehensive care plans.
  • Educate patients on the proper use and potential side effects of their medications.
  • Monitor patient compliance and response to therapy, adjusting prescriptions as necessary.
  • Participate in the practice's chronic disease management by supporting clinics such as diabetes or asthma clinics.
  • Implement and manage medication safety and quality assurance protocols.
  • Keep abreast of the latest clinical data and research to provide evidence-based care.
  • Assist in the training and support staff within the practice.

Key Requirements:

  • Degree in Pharmacy (MPharm) or equivalent.
  • Registration with the General Pharmaceutical Council (GPhC) as a pharmacist.
  • Previous experience in a primary care setting is highly desirable.
  • Strong understanding of the NHS framework and clinical governance.
  • Excellent communication and interpersonal skills, capable of working effectively in a multidisciplinary team.
  • Proven ability to manage multiple priorities in a fast-paced environment.
  • Evidence of relevant CPD

Job responsibilities

Job summary:

  • The role is for a Clinical Pharmacist to support primary care teams by providing practice based medical services during the working day, Monday to Friday.
  • The clinician will work as part of the GP practice team with close collaborative working with other members of the Multidisciplinary Team and support from GPs..
  • To provide holistic care for individuals including assessment, diagnosis and management
  • To refer and/or signpost patients at the practice to other allied health professionals for example community COPD nurses, Social prescribers etc.
  • The post holder will use advanced clinical and communication skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment.
  • Care occasionally may need to be provided in other settings as required.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations
  • Advise patients on maintaining health and refer to allied services when a health promotion need is identified.
  • Communicate at all levels within the multidisciplinary team ensuring an effective service is delivered
  • Maintain accurate, contemporaneous healthcare records appropriate to the consultation
  • Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high-quality patient care
  • Work within local policies and procedures
  • Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service
  • Achieve and demonstrate agreed standards of personal and professional development in order to meet the needs of the service

  • Experience of working within a multi-disciplinary team

  • Able to professionally and appropriately challenge differing opinions as a patient advocate.

Desirable

  • Experience of clinical supervision
  • Experience of working in Primary and Community Care

Knowledge

Essential

  • In-depth knowledge of pharmacotherapy and clinical pharmacology.
  • Understanding of health education/promotion.
  • Understanding of Confidentiality and the Data protection Act
  • Understanding of clinical governance and its significance and impact upon practice.
  • Ability to participate in audit and standard setting
  • Awareness of equality and valuing diversity principles
