Job description

This is a permanent, full time position required for 37 hours per week.

This is an opportunity to case-manage care leavers 18-25 years old, and to act as a personal adviser to 16-18 year olds.

You will work within our Child Care division. This team supports young people who are leaving the care system and preparing to live independently. Assessments are carried out and developmental care packages arranged to ensure their welfare and well-being.

Your role will involve contributing to assessments, developing and implementing plans and working with the young people to enable them to live independently. The young people have a wide range of abilities and needs and you will be expected to work with them individually to enable them to develop independent living skills. The work entails quite a lot of travelling around Pembrokeshire with the occasional trip outside the county within the UK. You will be expected to work closely with other team members and agencies. Accurate recording is a vital part of the job and good computer skills are essential as most of the recording is done electronically.

Educated to NVQ3 (or prepared to work towards it) with experience in Social Care, Youth Work, Health, Education or a similar field, you will be successful at working with young people giving them the confidence and abilities they need. You will need a car available for work, and the ability to work through the medium of Welsh would be an advantage.
