How you'll make a difference
What will you be doing?
- You will deliver inclusive personalised pathway plans to maximise young people’s independence, ensuring that relevant legislation, codes of practice, and good practice guides are implemented, and service standards are met.
- You’ll maintain contact with and deliver pathway plans in accordance with the statutory requirements of the Personal Advisor role.
- Daily you will provide support, specialist advice and guidance as well as advocacy on behalf of young people.
- You will liaise with contacts from different sectors, including health, housing, education, and voluntary sectors ensuring good communication to deliver an effective service.
- You will take necessary action in accordance with Safeguarding procedures to protect vulnerable young people.
- You’ll maintain confidential reports and case notes, record care plans and risk assessments in accordance with professional and Council guidelines.
What we need from you
- You will hold a relevant qualification to NVQ3 level or equivalent in health and social or considerable experience of working with young people in a professional environment such as residential social work, youth work, career advisory work, young people’s drugs work or mentoring.
- You can demonstrate knowledge of relevant legislation, regulations, and codes of practice as well processes and procedures related to leaving care.
- You will have good interpersonal and communication skills and an ability to build relationships and engage successfully with young people, their families and professionals.
- It is required that you have a valid Driving Licence and access to car with Business Insurance.
