The post holder will be responsible for: Reviewing clinical documentation using Docman IT system Working to agreed protocol the post holder will: Check medication list from discharge summary/consultant review matches current medication list on primary care record Process dosage changes on primary care record where instructed by consultant review Process stop/start of medications on primary care record where instructed by consultant review Escalate review of patients to clinical pharmacist where changes are unclear/current medication lists do not match. Send confirmation to patients that medications on their repeat list have been altered as per consultant review The Pharmacy Technician will not issue any medications. These will be issued by the practice team upon request by patient. Involvement in audit/quality improvement work
