About the Role
- Undertaking patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
- Carrying out medicines’ optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
- Supporting the clinical pharmacists.
- Providing expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.
- Managing shared care protocols and liaise with clinical pharmacists for more complex patients.
- Supporting initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.
- Supporting the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed.
- Implementing efficient ordering and return processes and reducing medication wastage.
- Providing training and supporting on the legal, safe, and secure handling of medicines, including the implementation of Proxy Access for Care Homes for their residents.
- Promoting of electronic Repeat Dispensing (eRD) and online ordering.
- Developing relationships with other pharmacy professionals and members of the multi-disciplinary team supporting integration across health and social care including primary care, community pharmacy, secondary care, and mental health.
- Supporting the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
- Supporting the PCN to deliver on IIF, QOF and locally commissioned enhanced services.
About the Candidate
The successful candidate will:
- be a registered Pharmacy Technician with the GPhC;
- hold a Diploma Level 3 in pharmacy services, or equivalent;
- have strong experience in either a community pharmacy or a primary care setting;
- be confident in analysing and presenting information to health professionals;
- have experience of implementing and promoting cost-effective strategies; have the ability to interpret complex clinical information and present it in suitable formats, to a variety of people from professional and non-professional backgrounds.
