We are recruiting to a variety of roles working as part of an innovative MDT. You will be working for Shropshire Community Health NHS Trust with your base at The Royal Shrewsbury Hospital.

As a registered Physiotherapist you will be experienced in working in a multi disciplinary / multi-agency health / social care environment with knowledge of IT basic administration systems including Microsoft Office Suite (Word, Excel etc).
You will be patient focussed and have the ability to carry out comprehensive assessments and be able to prioritise and make sound decisions, sometimes under pressure.

To provide holistic and person-centred care and support community patients in a subacute setting.
Responsible for the assessment, planning, delivery and evaluation of individualised patient care, working in partnership with patients, their carers and the multi-disciplinary team to ensure the patient’s physical, spiritual and emotional needs are met.
