About the Practice:

Role Overview:
The Practice Nurse will assume a pivotal role in administering comprehensive nursing care, ensuring patients receive optimal support for managing their health. Collaborating closely with a multidisciplinary team, the Practice Nurse will uphold the practice's commitment to evidence-based care and health promotion.
Key Responsibilities:
- Conducting nursing assessments and delivering clinical interventions according to established guidelines.
- Managing chronic conditions such as diabetes, hypertension, and asthma through regular monitoring and patient education.
- Administering vaccinations and immunizations in adherence to national schedules.
- Performing wound care, dressing changes, and minor surgical procedures as necessary.
- Assisting patients in implementing lifestyle modifications to enhance health outcomes.
- Maintaining accurate patient records using electronic medical record systems.
- Collaborating with GPs, healthcare assistants, and other team members to ensure coordinated patient care.
Requirements:
- Registered Nurse with current NMC registration.
- Previous experience in a Practice Nurse or similar primary care role is preferred.
- Proficiency in nursing assessments, vaccinations, and chronic disease management.
- Strong communication skills, capable of engaging patients from diverse backgrounds.
- Excellent organizational abilities and adept at managing multiple priorities.
- Familiarity with electronic medical record systems is advantageous.
