This is not a comprehensive list but outlines min. requirements Provide general nursing care to practice population who are referred or self-refer in the practice or in home setting including assessment, planning, delivery and evaluation of: Administer nebuliser Spirometry/respiratory testing Cervical cytology Blood pressure and other physiological measurements Help the practice develop and deliver specialist primary nurse led services including chronic disease management. Initiate the role of diagnosis for patients suspected to have a chronic disease e.g. diabetes, COPD, asthma and CHD, referring to other clinicians as appropriate Manage programs of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions. Maintain disease registers in liaison with administrative staff. Undertake annual review of patients understanding and ability to self-manage .Provide comprehensive travel health advice for patients and administering of travel vaccines Administer child and adult immunisations and vaccinations in accordance with national and local programs. Competent in anaphylaxis and resuscitation techniques. Ensuring safe storage, rotation and disposals of vaccines and drugs and that adequate stock levels are maintained Undertake autonomous comprehensive assessment of patient need and develop care plan in accordance with local and national protocols Make judgements on range complex issues requiring analysis, assessment and solution Deliver care to specialist groups in accordance with practice protocols and QOF guidance, local and national guidance Provide health screening to practice population Autonomously refer to multidisciplinary team for referral/ treatment/ discharge Visit patients in their homes to deliver primary nurse led services Safely use and manipulate equipment used in examination and treatments Computer literate with practice patient record system and electronic communication systems Safely manage hazardous biological substances including blood/urine/vomit/faeces/vaginal secretions Communicate effectively with patients, relatives and carers regarding treatment and progress of patients, maintaining confidentiality of all patient information Manage issues arising from sensitive situations e.g. unwanted pregnancy, sexually transmitted infection, child protection issues mental health Act as patient advocate/chaperone Awareness and provision of evidence based care Physically handle patients to assess/treat in emergency situations To provide home visits to patients.
