About the Role
Bildeston Health Centre has a fantastic opportunity for an experienced Practice Nurse to join our team on a full-time basis, for 37.5 hours per week,to be responsible for the implementation of processes for the effective management of patients with long-term conditions, using evidence-based practice including care for elderly and housebound patients.
The main responsibilities of the role include:
- acting as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing [or supporting the implementation of] audits, training, support and evidence of compliance to the practice manager.
- developing, implementing and embedding an effective care management programme for the frail and housebound patients registered at the practice.
- assessing the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
- developing, implementing and embedding health promotion and wellbeing programmes.
- implementing and evaluating individual treatment plans for chronic disease patients that promote health and wellbeing.
- identifying, managing and supporting patients at risk of developing long-term conditions, preventing adverse effects to patients’ health.
- as required, providing routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF.
- as required, diagnosing and managing acute and chronic conditions.
- prescribing medication for patients in accordance with PGDs and own scope of practice.
- reviewing patient medications to enhance compliance
- providing guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages
- co-ordinating the provision of travel medicine services.
- requesting pathology services and process pathology results as required.
- overseeing the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
- maintaining accurate clinical records in conjunction with extant legislation.
- ensuring the appropriate maintenance and use of chronic disease registers.
- developing, implementing, embedding and participating in well woman and well man clinics.
- assisting GPs with minor surgery when required.
- prioritising health issues and intervene appropriately.
- recognising, assessing and referring patients presenting with mental health needs.
- implementing vaccination programmes for adults and children.
- liaising with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- deliver opportunistic health promotion where appropriate
About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
- A relevant nursing degree.
- Current NMC registration with an active PIN.
- Experienceof working in primary care.
- Experience in chronic disease management.
- Experience in infectionprevention and control measures.
- Experience in ECGs, venepuncture, wound care, immunisations, & spirometry.
- Excellentcommunication skills.
