Job description

Job responsibilities You remain under the regulation of your professional body and must continue to comply with the relevant standards of proficiency; recognise and work within own competence and in accordance with the professional Code of Conduct of your original registered profession. Primary Responsibilities: Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health. Supporting and managing the health needs of women presenting for family planning and cervical cytology consultations Implement and participate in vaccination and immunisation programmes for both adults and children. Advise, support and where appropriate administer vaccinations for patients travelling abroad. Provide wound care to patients. Request and process pathology services as required. Provide holistic chronic disease care for patients by implementing and evaluate individual treatment plans, delivering and monitoring patient care and referring patients to specialist care as required. Maintain accurate clinical records in conjunction with professional code of conduct and legislation. Ensure read codes are used effectively. Maintain chronic disease registers. Recognise, assess, and refer patients presenting with mental health needs. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice) Support the clinical team with all safeguarding matters, in accordance with local and national policies. Deliver opportunistic health promotion where appropriate. Develop, implement, and embed health promotion and well-being programme. Support the team in dealing with clinical emergencies. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.) Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individuals). Involved in clinical audit, effectively utilising the audit cycle. Monitor and ensure the safe storage, rotation, and disposal of medicines. Support junior members of the nursing team to help them achieve their potential, providing guidance when necessary. Participate in local initiatives to enhance service delivery and patient care. Support and participate in shared learning within the practice. Continually review clinical practices, responding to national policies and initiatives where appropriate Secondary Responsibilities: Participate in planning and implementing changes within the area of care and responsibility. Uphold the values and behaviours of your Profession and act as a positive role model. Monitor the safety and effectiveness of own clinical practice through quality assurance strategies such as the use of audit, feedback from clinical managers and peers, case review and peer review. To ensure Risk Management requirements are adhered and any assessments completed on time. The post holder is responsible for ensuring they undertake the appropriate level of training in accordance with the safeguarding policy training strategy so that they ensure they promote the welfare of children and adults. The post holder must always comply with the practice Health and Safety policies, by following agreed safe working procedures and reporting incidents using the practice risk incident reporting system. Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards. All members of staff are bound by the requirements of the Data protection Act 1998. Whilst seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data. The post holder must co-operate with all policies and procedures designed to ensure equality of employment.
