The Practice Nurse will work as part of the primary healthcare team providing chronic disease management and primary medical services to patients. The Practice Nurse will take a lead role in coordinating and organising the practice recall system in line with QOF requirements. Patient Services New Patient Medicals Travel Vaccinations Cytology Asthma / COPD / Respiratory Management Hypertension, Diabetes, Coronary Heart Disease and Contraception Management Venepuncture Routine Immunisations and Childhood Vaccinations Request and process results of pathology tests for example urine culture / swabs and bloods Following agreed clinical protocols with referral to senior nurses / GP as appropriate Active Participation in administrative systems Attend and participate in practice meetings when required On occasion there may be a requirement to undertake a home visit Any other delegated duties appropriate to the post Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice) Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) Support the clinical team with all safeguarding matters, in accordance with local and national policies
