Provide care within own competence and seek advice or refer to colleagues when appropriate, ensuring that practice is effective, relevant, evidence-based and of a high standard. Provide face-to-face contact (at the patients residence when necessary) and follow up telephone consultations. Provide group consultation for the purposes of educating patients with similar conditions to strengthen proactive and preventative care. Assess patient needs holistically, taking into account co-morbidities and multi-factorial needs. Provide early intervention and self-management treatment. Provide risk assessment and report concerns to GP / ANP. Provide and advise on a range of treatment options including self-help and other interventions and referring to Mental Health support services. Plan care accordingly, integrating treatment methods into a management plan. Make appropriate referrals to other health care professionals including the organisations clinical team. Work with patients in order to support compliance with and adherence to management plans. Provide fast access to treatment for conditions that can be appropriately managed in Primary Care. Support patients to adopt healthy lifestyles and apply principles of self-care. Support families and carers to understand and manage patients Mental Health problems. Provide Mental Health reviews for appropriate patients to include physical health. Work supportively alongside clinical and administrative colleagues in ensuring the highest standard of care for all Practice patients. Maintain accurate, contemporaneous and correctly coded records to relevant professional and Organisational standards and guidelines. Develop, implement and lead projects as required. Develop Mental Health protocols in conjunction with the Clinical Leads. Act as a resource for the clinical team by providing expert advice, education, training and development for the clinical team to include referral pathways, self-management, health promotion and early intervention. Act as an expert resource to strengthen the practices approach to patient care regarding Mental Health management. Make decisions regarding patient treatment, to include requesting of diagnostic tests and use the results to support diagnosis and decision-making. Provide emergency care as necessary.
